Wrist Injuries in Tennis: Causes, Prevention, and the Grip Fix Most Coaches Miss
Wrist pain in tennis players is almost always caused by grip issues, poor contact points, or overtight strings. Here is how to prevent it.
Wrist Injuries in Tennis
Wrist injuries are second only to shoulder injuries in serious adult players — and the fix is almost always mechanical or equipment-based, not medical. Here's what causes them and how to prevent them.
Quick answer
Tennis wrist pain has four common causes: (1) death-gripping the racquet, (2) hitting late so the wrist has to compensate, (3) heavy topspin forehands with a Western grip and aggressive wrist snap, and (4) stiff polyester strings. Fix these and 90% of wrist pain resolves.
The four causes
1. Death-gripping the handle
Most club players hold the racquet at 8/10 pressure the entire point. That's exhausting for the forearm and dangerous for the wrist.
The correct grip pressure is 3–4/10 during rallies, tightening slightly at contact. The Vic Braden analogy: "hold the racquet like a bird — firm enough that it doesn't fly away, gentle enough that you don't crush it."
The fix: consciously relax your grip between shots. Loosen the fingers, let the racquet almost slip. Re-tighten only just before contact.
2. Late contact
If you hit the ball behind your front foot, your wrist has to bend and manipulate the racquet to make contact. That repetitive wrist manipulation under load is a straight path to TFCC injury (a small cartilage tear on the pinky side of the wrist).
The fix: contact point out in front of the body — one foot in front of your front hip for forehands. If you're always late, work on split-step timing and shorter backswings.
3. Aggressive wrist snap on topspin
Some players (especially self-taught adults) generate their topspin with a big wrist snap at contact. It works, but at the cost of long-term wrist health.
The fix: generate topspin from a low-to-high racquet head path driven by the shoulder and body rotation, not from wrist flex. The wrist stays firm through contact; the racquet head goes low-to-high because the arm swings that way.
4. Stiff polyester strings
The stiffest strings on the market (polyester) are the worst for wrists. Vibration transfers directly through the racquet handle into the wrist and forearm.
The fix:
- Switch to multifilament or hybrid strings
- If you must use poly, use it at low tension (48–52 lbs)
- Consider a softer frame (RA under 65)
Prevention exercises
Wrist curls: hold a 3–5 lb dumbbell, wrist over the edge of a table. Curl the wrist up (palm-up motion), then reverse curl (palm-down). 3 sets of 15 each direction.
Squeeze balls: 30 seconds squeeze, 30 seconds rest. 3 rounds daily. Simple but effective for grip endurance.
Wrist rotations: dumbbell held vertically, rotate wrist side to side. 3 sets of 15.
Forearm stretch: arm extended, opposite hand pulls fingers back gently. Hold 30 seconds each side.
Warm-up specifically for wrists
Before hitting:
- 10 wrist circles each direction
- 10 finger flexes
- 20 easy volleys with light grip pressure to warm up the whole complex
Cold wrists are stiff wrists.
Common warning signs
Stop and address:
- Pain on the pinky side of the wrist (TFCC red flag)
- Clicking or catching during forehand contact
- Grip weakness — trouble holding a coffee cup
- Numbness or tingling in the fingers
These indicate structural problems, not just soreness. See a hand/sports orthopedist if pain persists past 2 weeks.
Grip size
Wrong grip size is a hidden cause of wrist pain. Too small = you overtighten to control it. Too big = your fingers can't wrap enough to stabilize it.
Correct grip size: with your dominant hand around the handle, you should be able to fit the index finger of your other hand snugly between your fingertips and your palm.
Most adults are between grip size 3 (4 3/8") and grip size 5 (4 5/8"). If you're between sizes, err smaller — you can add overgrip to build up, but you can't shave a grip down.
Return to play after injury
Wrist injuries need patience. Even a mild strain:
- Days 1–14: rest, ice, no tennis. Splint if the doctor recommends it.
- Days 15–28: prehab exercises pain-free, still no tennis.
- Weeks 5–6: light rallies, no full swings, no serves.
- Weeks 7–8: full play if pain-free.
Playing through wrist pain is how a 3-week injury becomes a 6-month one. Respect the timeline.
